The employee wore a N95 dust mask respirator and was not provided with an annual fit test, on or about 10/21/2015.      ABATED DURING INSPECTION  NO ABATEMENT CERTIFICATION REQUIRED      Halliburton Energy Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 956824, citation number 01, item number 01, and was affirmed as a final order on 05/19/2014, with respect to a workplace located at 335 Cr 284, Harwood, TX 78632.
