29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  SHL Lures, LLC - Toccoa, GA -  Temporary and full-time employees were exposed to molten lead chemical splash hazard while adding, melting and scraping the impurities from the lead pot.  The employer did not conduct an assessment to determine the type of hazards located at the jobsite and the appropriate personal protective equipment to be utilized when working around the lead melting pot (400-700 degrees F) hazards, on or about 10/21/15.

29 CFR 1910.1025(d)(2): An initial determination was not made to determine if any employee may be exposed to lead at or above the action level;  a) Temporary and full-time employees were melting, pouring and scraping antimonial lead alloy (containing 90-99% lead and 0-10% antimony) to manufacture fishing lures on or about October 21, 2015.  The employer did not make a determination whether or not employees were exposed above the action level by conducting air monitoring.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
