29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Production Floor - On or about September 17, 2015, where employees perform service and/or maintenance on equipment with hazardous energy sources to include, but not limited to, Hydraulic Power Presses, Mechanical Power Presses, Bead and Trim Machines, Conveyor Washers, Lucas Curling Machine, and Welding Machines, the employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections. Abatement Note: 1. Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. Procedures shall meet the requirements outlined in 1910.147(c)(4)(i) and (ii). 2. Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. Hardware shall meet the requirements outlined in 1910.147(c)(5)(i) through (iii). 3. Periodic Inspection of the procedures shall be completed at least annually and certified. The periodic inspection shall be meet the requirements of 1910.147(c)(6)(i) through (ii). 4. Training shall be meet the requirements outlined in 1910.147(c)(7)(I) through (iv). 29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) Throughout Facility - On or about October 5, 2015, each employee who performs servicing and maintenance, to include set-up, on equipment with hazardous sources of energy, did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. Abatement Note: a. Training must meet the minimum requirements outlined in 1910.147(c)(7) to include: A. Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. B. Each affected employee shall be instructed in the purpose and use of the energy control procedure. C. All other employees whose work operations are or may be in an area where energy control procedures may be utilized, shall be instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out. b. The employer shall certify that employee training has been accomplished and is being kept up to date. The certification shall contain each employee's name and dates of training.
