29 CFR 1910.146(c)(8)(i): When an employer arranged to have employees of another employer (contractor) perform work that involved permit space entry, the host employer did not inform the contractor that the workplace contained permit spaces and that a permit was required. Jobsite: The host employer did not inform the contractor that the fuel tank was a permit required confined space and that an entry into that space was only allowed when there was a permit space program utilized. On or about 10/11/2015, an explosion/flash fire occurred while a contractor was in the process of performing grinding operation inside the tank with a hand-held electrical grinder, and the contractor sustained severe burns.
