This violation occurred on or about October 22, 2015, the specific written lockout/tagout procedures for equipment such as, but not limited to, the Roll-In Saw lacked the specific steps to verify the effectiveness of the energy control measures exposing employees to caught in and struck by hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. The employer did not ensure machine guarding was in place to protect employees from ingoing nip points and moving parts for the following machines: a) This violation was observed on or about October 22, 2015, the two (2) Cosen horizontal band saws, the unused portion of the blade was not guarded exposing employees to being struck by the moving blade. b) This violation was observed on or about October 22, 2015, the Roll-In Saw vertical band saw, the unused portion of the blade was not guarded exposing employees to being struck by the moving blade.
