29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: Employee break room, Core-Mark International, 355 Main Street, Whitinsville, Massachusetts; Electrical equipment in the employee break room had missing knockouts and breakers exposing employees to electric shock hazards. a. A 4 outlet receptacle box was missing a knockout exposing employees to a 120 volt electric shock hazard. b. Breakers were missing from a wall hung distribution panel that had the door open exposing employees to an electric shock hazard of 120 volts.
