29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 29 CFR 1926.501(b):    a. During residential roofing work, employees were exposed to a fall hazard 8 feet six inches, in that, four employees were stripping and installing roofing shingles without any type of fall protection, on or about 10/20/2015.

During residential roofing work, employees exposed to a fall hazard were not provided training and education relating to fall protection, hazard recognition, procedures to be followed to minimize these hazards and the proper use and limitations of fall protection equipment and fall protection systems, on or about 10/20/2015.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. During residential roofing work, employees were exposed to a fall hazard 8 feet 6 inches, in that, an extension ladder used to gain access to the roof extended 2 feet 6 inches from the landing surface, on or about 10/20/2015.
