29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: On or about December 4, 2015 it was mandatory for employees to wear a filtering face piece respirator when working in the milling room. Employees wearing a respirator were not provided: a medical evaluation, procedures for proper use during routine and emergency situations, fit testing; training on respiratory hazards, training on proper use, donning and removing the respirator, procedures for cleaning, storing and disinfecting.
