29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:    Employer did not provide certification on each of the cited items:    (a)  Frequent and regular inspections of the job site are being completed.    (b)  Fall protection training has been given to untrained employees.    (c)  Fall protection systems are being used when required during residential construction.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a) The employer did not ensure that a competent person was conducting frequent an regular inspections of the work sites to ensure employees were utilizing the appropriate safe guards, such as but not limited to fall protection. This exposed employees to a fall hazard of approximately 17 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) The employer did not institute a training program for employees which are likely to be exposed to fall hazard; which included hazard recognition and means to abate or minimize those hazards. This exposed employees to fall hazards when conducting roofing work.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) The employer did not ensure that employees conducting roofing work on a residential building were protected from falling to the lower level by means of an appropriate fall protection system. This exposed employees to a fall hazard of approximately 17 feet.
