29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:     a)  Reliant Holdings, Inc. at 31375A Great Western Drive, Windsor, CO 80550:  On or before October 22, 2015, the employer did not ensure that employees were protected from amputation hazards when working with the Airport Cut (APC) ice machine serial number 051231.  An employee was sweeping away ice pellets from the point of operation with the guard raised and with the machine in operation, when the hydraulic press amputated the tip of his finger.  Condition exposed employees to an amputation hazard.
