29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.   a. On Edgewater Drive, west of the intersection with Midway Blvd, employees of Stevens and Layton, Inc. were exposed to a struck by hazard while installing a 10 inch sanitary sewer pipe and a 12 inch reclaim water pipe, in that, excavators were driven on top of the spoil placed right at the edge of the north excavation wall which was about 10 feet deep and 250 feet long, on or about 10/21/2015.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraphs (b) or (c) of this section:   a. On Midway Blvd, at the south side of the intersection with Edgewater Drive, employees of Stevens and Layton, Inc. were exposed to possible cave-ins, in that, shoring or sloping was not used to protect employees when installing a 10 inch sanitary sewer pipe in a trench excavation with type "C" soil which was 10 feet deep, 45 feet long and 17 feet wide, on or about 10/21/2015.   b. On Edgewater drive west of the intersection with Midway Blvd, employees of Stevens and Layton, Inc. were exposed to possible cave-ins, in that, shoring or sloping was not used to protect employees when installing a 10 inch sanitary sewer pipe and a 12 inch reclaim water pipe in a trench excavation with type "C" soil which was 10 feet deep, 250 feet long and 15 feet wide, on or about 10/21/2015.
