29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    On or about 10/22/2015, Mile Square Rd.    a) The three employees were installing gutters from the ladder jack scaffold. The scaffold was 16' high and the employees were not protected by a personal fall arrest system.

REPEAT VIOLATION    Anthony's Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.21(b)(2), which was contained in OSHA inspection number 403642, citation number 1, item number 1 a and was affirmed as a final order on 6/26/2012, with respect to a workplace located at 3624 Irwin Ave., Bronx, NY 10471.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about 10/22/2015, Mile Square Rd.    a) A laborer was working below a ladder jack scaffold cleaning roof debris. He was not wearing a hard hat while other employees worked above.                                                                REPEAT VIOLATION    Anthony's Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 403642, citation number 1, item number 1 b and was affirmed as a final order on 6/26/2012, with respect to a workplace located at 3624 Irwin Ave., Bronx, New York 10471.
