29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about October 22, 2015, lockout/tagout was not used for the following processes:  a. The employee entered the crosslapper in order to perform re-threading and was exposed to ingoing nip points when the interlock on the door was used to stop the machine.   b. The employee at the winder is exposed to ingoing nip points when he gets onto the machine in order to start the next roll by hand.  The machine is stopped at the winder control panel.    c.  Employee was exposed to struck-by and electrical hazards when procedures were not developed for maintenance operations such as, but not limited to, replacing a motor or a hose.    d.  Employees locked out the emergency stop when performing cleanup and preventive maintenance.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them.  On or about October 22, 2015, the lockout/tagout procedures for equipment with multiple energy sources did not specify the order that the locks were to be removed in order to re-energize the equipment.  Equipment such as, but not limited to, the Line 1 Loom Plant Air Supply have multiple energy sources.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about October 22, 2015, employees were exposed to caught-in hazards when the following were not guarded: a. loom exit (material entering four roll stretch)--ingoing nip point b. scray rollers (rollers between calender and scray)--ingoing nip point c. there was a gap at the bottom of interlocked door to the crosslapper--ingoing nip point   No abatement certification or documentation is required for this item.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxillary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substanially equivalent to the wall of the equipment:  On or about October 22, 2015, employees were exposed to electrical hazards when the electrical outlet used to provide power to the computer equipment located next to the winder was missing several knockouts. 29 CFR 1910.1200(h)(2)(iii): The employer did not provide information to the employees as to the location and availability of the written hazard communication program, and material safety data sheets required by 29 CFR 1910.1200:  On or about October 22, 2015, employee did not know the location of the safety data sheets at the site.  Employees knew that the SDS described hazards associated with chemicals.  Employee used chemicals such as, but not limited to, propane and diesel.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
