29 CFR 1910.178(o)(2):  Loads greater than the rated capacity of the forklift were handled.  a.  On or about 10/13/15, in the rebuild and assembly areas:  An extruder base and accompanying equipment were handled by two forklifts having 5,000 and 7,000 pound capacities and a five ton crane.  The load handled exceeded the rated capacity of the forklifts.  Abatement certification must be submitted for this item.

29 CFR 1910.179(n)(1): Crane(s) were loaded beyond the rated load (maximum load for which the crane or individual hoist was designed and built by the manufacturer and shown on the equipment nameplate.  a.  On or about 10/13/15, in the rebuild and assembly area:  A DeShazo five ton overhead crane was loaded beyond its rated capacity in that it was used to lift an extruder base and accompanying equipment which weighed 35,000 pounds.  Abatement certification must be for this item.
