29 CFR 1926.405(a)(2)(ii)(E): All lamps for general illumination were not protected from accidental contact or breakage: In the generator room, flexible cords with 110 VAC light bulbs were laying on the floor. The metal cage around the light bulbs was completely open at the bottom. Employees that worked in the area with ladders, scaffolds, and other tools were exposed to the hazard of contacting electricity.

29 CFR 1926.405(j)(1)(i): Fixtures, lamp holders, lamps, rosettes, and receptacles shall not have live parts normally exposed to employee contact: (a) In the generator room, flexible cords for 110 VAC light bulbs did not have light bulbs in every socket. Portions of the cords were laying on the floor. The metal cage for the light bulbs was completely open at the bottom. Employees that worked in the area with ladders, scaffolds, and other tools were exposed to the hazard of contacting electricity. (b) In the northeast corridor, a 110 VAC light bulb was broken and the energize filament was exposed on a flexible cord with multiple light bulbs. Employees who worked next to the bulb to apply joint compound to drywall were exposed to the hazard of contacting electricity.

29 CFR 1926.417(b): Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized: Near the northeast corner of the project near the corner of 10th Street and Cromwell Street in temporary panel B, a 240 VAC 60 ampere circuit breaker was in the off position. There were no locks or tags attached to the breaker. The label for the breaker showed the breaker was used to supply power to a trailer onsite. The flexible cord from the breaker to the trailer was cut off about four feet from the temporary panel, and the cut end laid on the ground. Employees that walked near the panel to access their vehicles in a parking area were exposed to the hazards of contacting electricity.
