29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On the east side of the building, an extension cord that was providing electrical power to various power tools had no strain relief, allowing the outer sheathing to be pulled back with the internal wiring exposed, exposing employees to electrical shock hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On the east side of the building, employees were working on a steep roof without using any type of fall protection, exposing employees to falls of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed:  On the east side of the building, the aluminum extension ladder used by employees to access the work area did not have side rails extended at least three feet above the edge of the roof.  Employees were exposed to falls of approximately 16 feet to the concrete below.
