29 CFR 1910.28(j)(4): The worker was not protected by a safety life belt attached to a lifeline securely attached to substantial members of the structure (not scaffold) or to securely rigged lines, which would safely suspend the worker in case of a fall: 1200 Crown Colony Drive, Quincy, Ma. One rope was used for both the working rope of the boatswains chair and for a lifeline and both parts were connected to the roof mule.
