Employees were exposed to the hazards of electrical shock and burn.

29 CFR 1926.405(g)(1)(ii): Attachment plugs for cords. If used as permitted in paragraphs (g)(1)(i)(C), (g)(1)(i)(F), or (g)(1)(i)(H) of this section, the flexible cord was not equipped with an attachment plug and was not energized from a receptacle outlet. (a) Naval base, San Diego, water side, Building B127: One yellow flexible temporary cord was not connected to an attachment female plug, as the three conductors within the cords were directly wired to the switch. The employees were exposed to the hazards of electrical shock and burn.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems: (a) Naval base, San Diego, water side, Building B127: One employee stood near the edge of the roof using a rope to lower a five-gallon plastic bucket filled with tools without wearing any fall protection and was exposed to an approximately 21-foot fall to the ground below. The condition made it highly likely an accident could have occurred and that any such accident could have caused severe bodily injury or death.
