29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) Colleen Enterprises, LLC on or about October 27, 2015, at the address of 27385 Carronade Dr. Perrysburg, OH: The employer failed to ensure employees working from an aerial lift were adequately protected from falls. Two employees working from the lift were not clipped off to the basket while the platform was elevated. Employees operating in this condition were exposed to falls up to 25 ft. to the pavement below.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. (a) Colleen Enterprises, LLC on or about October 27, 2015, at the address of 27385 Carronade Dr. Perrysburg, OH: The employer failed to ensure employees working from Genie model Z-60/34 aerial lift were trained to recognize and avoid hazards associated with this type of scaffold system, such as, but not limited to fall hazards.
