29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) The employer did not ensure that employees were provided with adequate training on the appropriate techniques and procedures for controlling hazardous energy, and the recognition of hazardous conditions which require lock-out/tag-out. This exposed employees to amputation and avulsion hazards when conducting servicing/maintenance activities on the 4260 Automatan lamination machine.
