29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures was not permanent and continuous: (a) Naval base dry side, Snyder Hall Building 3380, Room 214: One CEP ground-fault circuit interrupter (GFCI) was missing a grounding prong pin of a 3-prong plug. The pin was broken off. One air filter system OMNI AIRE 1000VX was connected to the GFCI. The air filter system did not bear the double insulated symbol (a square inside another square), exposing worker to electrical shocks, burns, and/or electrocution.
