29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:   a) 2730 Severn St., Metairie, LA; On or about October 28, 2015, the employer allowed two employees to work from a platform being raised by a forklift that was not designed by a qualified person. The employee was approximately 22 feet above the ground.
