29 CFR 1928.57(a)(6): Operating instructions. At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved. On or about 25 October 2015 and at times prior thereto, the employer did not ensure that all employees operating the cotton module feeder were trained at least annually on the operating and maintenance procedures, exposing the worker to the hazard of getting caught in the moving parts.

29 CFR 1928.57(a)(8)(i): Where guards were used to provide the protection required by this section, they were not designed and located to protect against inadvertent contact with the hazard being guarded. On or about 25 October 2015, and at times prior thereto, the power transmission area for the cotton module feeder was not guarded to prevent inadvertent contact with the moving belts and pulleys, exposing the employees to the hazard of being caught in the rotating belts.
