29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use. On or about October 21, 2015 at 1339 Laney Walker Blvd., Augusta, GA., three employees installing a metal roof were observed using a job made basket to access/egress the working level. The employer used a job made basket that was not approved by the manufacturer to use with their rough terrain forklift.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel. On or about October 21, 2015 at 1339 Laney Walker Blvd., Augusta, GA., four employees were observed being lifted to the working level in a basket that was not firmly secured to the back rest or forks of the rough terrain forklift.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about October 21, 2015 at 1339 Laney Walker Blvd., Augusta, GA., three employees were observed installing a metal roof working a the leading edge exposed to a fall approximately 22'-8" to the lower level. At the time the Compliance Officer observed the employees, all on the working level were wearing their body harness and lanyards, but there was no anchorage point provided to the employees to tie off too.
