29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  (a)  Frequent and regular inspections of the job site, materials and equipment to be made by a competent person were not made as indicated by:          1.  Workers accessed the roof using an extension ladder that did not extend at least three feet beyond the roof's landing.      2.  Workers accessed the roof using an extension ladder that was erected over and near energized power lines.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that did not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:  (a) At the 2505 N 11th work site the approximate 32 foot aluminum extension ladder was not extended at least 3 feet above the upper landing.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive side rails:  (a) At the 2501 N 11th work site, workers were accessing the roof using an aluminum extension ladder which was erected over and within approximately 3 feet of a live 220 volt power line.  The ladder did not have nonconductive side rails.
