29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. Location: Asphalt plant On or about 10/29/15, the employer did not maintain the passageway and employees were exposed to trip hazards in the following but not limited to areas; 1. Bottom of staircase to silo - large pieces of asphalt material in front of staircase. 2. Main truck entrance/exit - hole in drainage grating. 3. Detention pond - man-made drainage ditch. 4. Parking lot area - hole in drainage grating covered by ply-board. 5. Incline conveyor - pile of asphalt dirt and debris in passageway.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Location: Asphalt Plant On or about 10/29/15, the employer did not provide a double swing gate on the following work platforms that employees accessed without fall protection; 1. Ladder #9 - the emulsion tank platform's fixed vertical ladder was not equipped with a double swing gate and had an opening where employees were exposed to fall hazards of up to 8-feet, 11 and 3/4-inches to the concrete ground below. 2. Ladder #10 - Burner platform's fixed vertical ladder was not equipped with a double swing gate and had an opening where employees were exposed to fall hazards of up to 10-feet, 8-inches to the concrete berm below.

Location: Silo platform, fixed ladder #13 On or about 10/29/15, employees were exposed to fall hazards of approximately 12-feet, 2-inches when transitioning from the fixed vertical ladder to the platform that was greater than 12-inches away from the ladder.
