29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  At the jobsite located at 805 E Main St., Independence, Kansas 67301, employees performing roofing work were exposed to fall hazards of approximately 12 feet to the ground below in that a fall protection system was not used.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite located at 805 E Main St., Independence, Kansas 67301, employees performing roofing work were exposed to fall hazard of approximately 12 feet to the ground below in that the employer did not train employees in appropriate fall protection methods to avoid hazards associated with elevated work areas.

29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder.  At the jobsite located at 805 E Main St., Independence, Kansas 67301, employees performing roofing work were exposed to fall hazards of approximately 12 feet to the ground below in that employees accessed the roof using ladders which did not extend 3 feet past the upper landing surface.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  At the jobsite located at 805 E Main St., Independence, Kansas 67301, employees performing roofing work were exposed to fall hazards of approximately 12 feet to the ground below in that were not trained to recognized damaged ladders and safe ladder placement for access to working surfaces.
