29 CFR 1910.272(g)(4): The employer did not provide equipment for rescue operations, which was specifically suited for the bin, silo, or tank being entered; At the time of the inspection, it was determined that employees were entering the top of grain bins for sampling and the employer had no equipment to provide rescue if the employee were to encounter an engulfment hazard.

29 CFR 1910.305(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: At the time of the inspection, the following boxes were missing covers and unused openings were not effectively closed; a. The opening on the grain dryer main panel box exposed employees to live electrical. b. The openings in the bin #9 fan disconnect box exposed employees to live electrical. c. The conduit box in the #8/9 pit was missing a cover exposing employees to live electrical. d. The motor starter control for the auger for pit #3 was missing the cover exposing employees to live electrical. e. There were multiple instances where conduit was damaged and employees were exposed to live electrical.
