29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a. On or about October 29, 2015, on the production floor of the facility, employees operating forklifts and employees on foot were required to share the same permanent aisles and passageways. The employer did not ensure that the aisles and passageways were appropriately marked. Employees were thereby exposed to struck by hazards.

29 CFR 1910.178(n)(6): The driver shall be required to look in the direction of, and keep a clear view of the path of travel. a. On or about October 29, 2015, on the production floor of facility, employees were required to operate forklifts on a daily basis. The employer did not ensure that all forklift drivers look in the direction of travel when driving in reverse. Employees were thereby exposed to struck-by hazards.
