29 CFR 1910.132(f)(1)(i): The employer did not provide training to each employee who is required by this section to use PPE on knowing when PPE is necessary:  (a)  On October 20, 2015, at the Saint Joe Distributing facility, employees were exposed to serious fall injuries, in that the employer had not provided Fall Protection PPE Training to all High Lift Powered Industrial Truck Operators.      Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.178(l)(2)(iii): All operator training and evaluation was not conducted by persons who have the knowledge, training, and experience to train powered industrial truck operators and evaluate their competence:  (a)  On October 20, 2015, at the Saint Joe Distributing facility, employees were exposed to serious injuries, in that the employer did not ensure that a competent person having the knowledge, training and experience was conducting training and evaluations to the PIT Operators as required by this standard.     Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.178(l)(6): Certification. The employer had not certified that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:  (a)  On October 20, 2015, at the Saint Joe Distributing facility, the employer had no records of certification showing that Powered Industrial Truck Operators had been trained and evaluated.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition:  (a)  On October 20, 2015, at the Saint Joe Distributing facility, employees were exposed to serious struck-by injuries, in that the defective equipment noted as follows:      (1) CAT Powered Industrial Truck # 8587X - in operation with a defective foot brake and a non-operational emergency brake.      (2) Raymond High Lift - in operation with defective warning light.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
