29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   a. 3051 Kumho Parkway, Macon, GA -  (Shipping Warehouse Canopy) - Employee was exposed to fall hazard. An employee was operating an aerial boom lift, elevated approximately 32-feet above ground level to the edge of the shipping warehouse canopy to take measurements of the area.  The employee was not utilizing attached to the basket while operating the aerial lift, on or about December 15, 2015.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a.  3051 Kumho Parkway, Macon, GA 31216  (roof of the new shipping warehouse) - Employee was exposed to fall hazard.  The employee was working (leaning over) and walking in close proximity to the edge of the flat roof canopy while performing roofing work and/or taking measurements, approximately 32-feet above ground.  The employee was not utilizing any type of fall protection, on or about 12/15/15.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a.  3051 Kumho Parkway, Macon, Ga. (Curing building pit area) - Employees were exposed to fall hazard.  Employees were walking and working in close proximity to an unguarded pit while being used as spotters for people who were designated to operate moving machinery in the area of the pit, exposing employees to fall hazard of 7-10-ft. from ground level to the inside of the pit, on or about 11/19/15.
