29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) 4605 Jonestown Rd, Harrisburg PA - An employee was observed working from an articulating aerial boom lift (Snorkel) and did not don a harness or body belt with lanyard attached to the boom or basket and was exposed to an approximate 30 foot fall hazard to the ground below, on or about October 30, 2015. b) 4605 Jonestown Rd, Harrisburg PA - An employee was observed working from a telescoping boom truck (Altec) and did not don a harness or body belt with lanyard attached to the boom or basket and was exposed to an approximate 25 foot fall hazard to the ground below, on or about October 30, 2015. No abatement certification or documentation required.

29 CFR 1910.67(c)(2)(ii): Aerial lifts were not operated by trained persons: a) 4605 Jonestown Rd, Harrisburg PA - Employees operating aerial lifts were not adequately trained in that they did not receive instruction in the recognition and avoidance of fall hazards, on or about October 30, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
