29 CFR 1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure; a. The ink screen wash up room - On or about November 2, 2015, several extension cords were being used as permanent wiring to power electrical equipment for employees to use during ink washing operations. Employees were thereby exposed to potential electrical hazards. The electric equipment and their conditions were the following; 1. An extension cord was placed between a floor fan's cord (off) and a power strip device which cord was plugged into an electric outlet located in the next room. 2. An extension cord was placed between a wall fluorescent light fixture's cord (on) and a power strip device which cord was plugged into an electric outlet located in the next room. 3. An extension cord was placed between a wall fluorescent light fixture's cord (off) and a power strip device which cord was plugged into an electric outlet located in the next room.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. a. The ink wash up room - On or about November 2, 2015, employees were required to conduct washing and rinsing operations to screen frames with safety glasses and gloves and the employer had not certified that a hazard assessment on personal protective equipment was verified at the workplace.
