29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:        Collins Roofing, Inc. had employees exposed to 12 ft. fall hazards, while accessing and exiting the roof via an extension ladder that only extended 1.5 ft. above the upper landing surface of the roof, on a residential construction site in Lincoln, Illinois.    The employer did not provide construction safety training to employees, in order to provide them with the knowledge necessary to recognize the hazards related to ladders, as well as the proper work procedures to follow, in order to minimize and eliminate theses hazards.  This training would include:      a. The nature of fall hazards in the work area,    b. The correct procedures for erecting, maintaining, and disassembling the fall protection systems to be used,   c. The proper construction use, placement, and care in handling of all ladders,   d. The maximum intended load carrying capacities of ladders, and    e. The standards contained in this subpart, Subpart X  Stairways and Ladders.

These fall hazards included the following:      a. 12 ft.  front of roof and    b. 17 ft.  side and rear of roof.    Collins Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 314848003, citation number 1, item number 3A and was affirmed as a final order on December 14, 2010, with respect to a workplace located at 820 Cherry Street, Hopedale, IL 61747.    Collins Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 77374, citation number 1, item number 2A and was affirmed as a final order on August 17, 2011, with respect to a workplace located at 1403 Winterberry, Bloomington, IL 61704.
