On or about October 19, 2015, at the above addressed job site, the employer did not provide a specific LOTO procedure for the Sandiacre Model TG 320 bagging machine which had pneumatic and electrical energy sources. An employee suffered the amputation his right index finger while adjusting the alignment on a bagging machine which was energized, running and had not been Locked Out.
