(a)  Located in the bilge of the USNS Henry Kaiser, moored at berth 315 at the Vigor Shipyard at 5555 N Channel Ave in Portland, Oregon: On or about and at times prior to October 29, 2015 the inspection revealed that the employer permitted respirators to be worn by employees who had facial hair that came between the sealing surface of the facepiece and the face.
