29 CFR 1910.28(a)(11): All planking or platforms shall be overlapped (minimum 12 inches) or secured from movement:    (a) On or about October 28, 2015, and at times prior to that date, Public Service Company of Colorado, dba Excel Energy, as the controlling employer, did not conduct frequent and regular inspections of the jobsite, materials and equipment in the Comanche #3 boiler so as to ensure that employees of Babcock & Wilcox Construction Company, were protected from fall hazards that were present at the site including but not limited to, fall hazards from job made scaffolds, lack of inspections of scaffolds by a competent person, access of the scaffolds in an unsafe manner, and planks not being secured from movement.      Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
