29 CFR 1926.451(b)(4): Each end of a platform, which was not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches:    (a)  On or about October 28, 2015, and at times prior to that date, employees were exposed to falls in excess of 10 feet where they were working from job made scaffolds where the working platforms were not adequately secured and the ends did not extend at least 6 inches over the centerline of the supports.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:    (a)  On or about October 28, 2015, and at times prior to that date, employees were exposed to falls in excess of 10 feet where they were accessing job made scaffold platforms, which were in excess of 2 feet below the access point, without the benefit of a ladder or other adequate means of accessing the working surface.
