29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a)   110 Route 10 West, Whippany, NJ.    Employee assisting in the installation of aluminum wall panels from the basket of an aerial lift was exposed to being catapulted out of the basket of the Skyjack telescopic boom lift as fall protection or restraint was not being used.                          Violation was observed on or about 10/31/2015.

Employee installing aluminum wall panels was exposed to falls of approximately 16 feet to the ground below as no method of fall protection was in use.                        Violation observed on or about 10/31/2015.    b)  110 Route 10 West, On the Roof, Whippany, NJ.    Employees installing aluminum wall panels were exposed to falls of approximately 16 feet to the ground below as proper fall protection was not being used.                        Violation occurred on or about 11/02/2015.
