29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about 10/29/2015 at the 7 North Street Construction Project, an employee was using a personal fall arrest system while working from an aerial lift and the lanyard was not attached to the employee's D ring located at the back and shoulder level of the harness.
