29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6m) above or below a point of access the employer did not provide one of the following; portable ladders, hookon ladders, attachable ladders, stair towers (scaffold stairways/towers), stairwaytype ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold.  Crossbraces shall not be used as a means of access:    a) Employees were exposed to 50 ft fall hazard when erecting a tubular welded frame scaffold, when climbing up and down the scaffold to get to the work area at the top of the scaffold. 29 CFR 1926.451(g)(2):  The employer did not have a competent person determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds. Employers are required to provide fall protection for employees erecting or dismantling supported scaffolds where the installation and use of such fall protection is feasible and does not create a greater hazard:    a) The employer did not ensure that employees use fall protection (FP harness with lanyard tied to the scaffold) when feasible when erecting a supported scaffold. Employees were not tied off when standing at the top of the well-wheel hoist (elevation 50 ft) unloading scaffold equipment.     b) The employer did not ensure that employees were equipped and able to use their personal fall protection systems. Several employees who were erecting the 50 ft tall tubular welded frame scaffold did not have lanyards on their FP harnesses.    Location: 560 7th Ave, New York, N.Y. 29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    a) The employer did not provide effective fall protection training for employees who erect and dismantle scaffolds.  Three employees erecting the 50 ft tall welded frame scaffold were working without FP lanyards.       Location: 560 7th Ave, New York, N.Y.
