29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented. Worksite located at 10354 S. 124th Street, Papillion NE: Employees were observed working on residential roof performing roofing activities without any type of fall protection system in use. This exposed the employees to falls ranging between 12 and 17.5 feet.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. Worksite located at 10354 S. 124th Street, Papillion NE: An extension ladder used for access and egress to roof did not extend at least 3 feet above the surface being accessed. The ladder was also not secured at its top to a rigid support.
