29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, and the platform did not have guardrails which block employee access to the cantilevered end;    35 Mason Street, Worcester, MA: On or about November 2, 2015 employees were working from a scaffold which had work platforms that extended more than 18 inches over the end support exposing employees to fall hazards.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    35 Mason Street, Worcester, MA: On or about November 2, 2015 employees were working from a pump jack scaffold which was not braced with intermediate braces as required by the manufacturer when in excess in 24 feet in height and the scaffold was also not braced at the bottom as required.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    35 Mason Street, Worcester, MA: On or about November 2, 2015 employees were working from a scaffold which was not equipped with guard rails, nets or any other acceptable form of fall protection exposing employees to fall hazards over 16 ft.    Construction for You, LLC was previously cited for a violation of this occupational safety and health standard 1926.451(g)(1) or its equivalent, which was contained in OSHA inspection number 716943 , citation number 1, item number 1b and was affirmed as a final order on January 22, 2013, with respect to a workplace located at 836 Main Street Worcester, MA 01607.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
