Sheffield Lake, OH: The employer failed to ensure a ladder or similar means of access was provided to employees using a welded frame scaffold. The employees were exposed to a fall to the ground below when utilizing the frame and crossbrace members of a masonry walkthrough scaffold to climb to the platform above.

The endpoints at each upright were more than 48 inches (1.3 m) apart.    a) Linton Masonry LLC on or about November 03, 2015, at the address of 5291 Detroit Rd. Sheffield Lake, OH:  The employer failed to ensure employees were protected from falls while working from a masonry walkthrough scaffold system. In addition to the rear crossbracing substituting for the top rail, the employer had not installed a midrail prior to permitting employees to operate atop the second frame platform. Employees performing masonry work in this condition were exposed to a 14 ft. fall to the ground below.
