29 CFR 1926.451(b)(1): Where the employer made a demonstration that a space wider than 1 inch was necessary between adjacent plank units and the platform and uprights, the platform was not planked or decked as fully as possible.   On or about November 2nd, 2015, at the jobsite located at 5870 S University Dr., Davie, FL 33328, an employee was exposed to a fall of approximately 12 feet while working from a scaffold that was not fully planked while performing plastering operations.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about November 2nd, 2015, at the jobsite located at 5870 S University Dr., Davie, FL 33328, an employee was performing plastering operations without the use of fall protection, being exposed to a fall hazard of approximately 12 feet.

On or about November 2nd, 2015, at the jobsite located at 5870 S University Dr., Davie, FL 33328, the employer did not provide the appropriate well-fitting gloves (butyl or nitrile) to employee who was not wearing any hand protection while using wet cement in the plastering operation.
