29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: At the north side of a residential home, employees access the tubular frame scaffold without a means of egress, exposing them to fall hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On the fourth tier scaffold platform, located at the north side of a residential home; employees worked from a tubular welded frame scaffold without end rails at the open ends at an approximate height of 20 feet, exposing employees to fall hazards.
