29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage must be made:    a) World Kitchen Warehouse Area - The employer did not evaluate the workplace to ensure an under ride hazard did not exist in floor level storage rack slots including but limited to Block 1 and Block 2 racks. Employees operated a JLG 20 MSP standup Mobil Stock Picker with a operator's compartment 60.5 inches above the floor in the racking area with horizontal beams 77.5 inches above the floor.  This would allow the horizontal rack beams at some storage locations to enter the operator compartment of the Mobil Stock Picker, pinning the operator between the horizontal rack beam and the mast of the machine. An employee was injured when they ran the machine ran under the horizontal beam and tipped over, on or about November 3, 2015.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.    World Kitchens LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.176(a), which was contained in OSHA inspection number 314247610, citation number 1, item number 1a and was affirmed as a final order on December 21, 2011, with respect to a workplace located at 1200 South Antrim Way Greencastle, PA 17225.  -----Cit 01 Item 001  violation modified from Repeat to Serious penalty modified from $14,000 to $7000 standard modified from 29 CFR Section 1910.176(a) to OSH ACT of 1970 Section (5)(a)(1) the alleged violation description to be amended as indicated below.    a)	World Kitchen Greencastle Warehouse Area - The employer did not evaluate the workplace to ensure a potential under ride hazard did not exist in the JLG 20 MSP standup Mobil Stock Picker in floor level storage rack slots including but limited to Block 1 and Block 2 racks. Employees operated JLG 20 MSP standup Mobil Stock Picker with an operator's compartment 60.5 inches above the floor in the racking area with horizontal beams 77.5 inches above the floor.  On or about October 29, 2015, an employee (who had been properly trained on the operation and use of the JLG MSP standup Mobil Stock Picker) was injured when she ran the JLG 20 MSP standup Mobil Stock Picker Model #20MSP, S/N 0130020023, MFG Date 2013 under the horizontal beam in Block 2 floor level storage racks and tipped over.  ---------------------------------------------------------------------------------------------------------------------------------
