29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a)    Chiller Room - On or about October 28, 2015 a procedure was not developed and utilized for the isolation of an anhydrous ammonia line going to the chillers prior to valves being shut on the line to prepare for an addition to the system.  Liquid anhydrous ammonia became trapped in the line between closed valves and caused a line rupture due to thermal expansion resulting in employees being exposed.    ABATEMENT CERTIFICATION REQUIRED
