29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Chiller Room - On or about October 28, 2015 a procedure was not developed and utilized for the isolation of an anhydrous ammonia line going to the chillers prior to valves being shut on the line to prepare for an addition to the system. Liquid anhydrous ammonia became trapped in the line between closed valves and caused a line rupture due to thermal expansion resulting in employees being exposed. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: (a) Chiller Room: On or about October 28, 2015 an employee that was responsible for conducting maintenance and operational activities with the anhydrous ammonia refrigeration system was not informed that the liquid anhydrous ammonia that is present in parts of the piping system can become a hazardous energy source in that it can expand and rupture the piping if trapped in the line. ABATEMENT CERTIFICATION REQUIRED
