29 CFR 1926.503(c)(3): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee. 29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  a)  On or about 11-02-15 at 1832 Eldorado Drive in Green Bay, each employee working on a roof was using a lifeline wrapped around a truss to anchor their vertical lifeline assembly, and not to a compatible anchorage connector.  The lifelines were not designed for use as anchorage equipment in fall arrest systems.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a)  On or about 11-02-15 at 1832 Eldorado Drive in Green Bay, employees were incorrectly using personal fall arrest equipment which would have created a greater than six (6) ft. free fall.  The employees were not making continuous adjustment of the rope grab device to reduce slack and prevent free fall to less than six (6) feet.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  a)   On or about 11-02-15 at 1832 Eldorado Drive in Green Bay, employees did not inspect their personal fall arrest equipment prior to use.  Employees were using damaged lifelines which showed deterioration and the lifelines were not removed from service.
