29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 11/3/2015, Machine #26 had an exposed slitter blade and ingoing nip points on the draw rolls with guards out of adjustment. Nip points were not protected from the bottom on the beaver tails where the napkins are transferred. b) On or about 11/3/2015, the TMC wrapper in-feed safety interlock switch was defeated. c) On or about 11/3/2015, the Galbreath Baler was operational with the guard door open.
